Establishing
an account and ordering tests through DLS
- Fax the completed HealthCare Provider Registration
form to our toll-free fax 1-800-728-9048 or submit
online.
- You will receive a HCP account code to
be used when placing all orders for your patients. You
may also setup a username and password to order online.
- You may order by telephone (800-908-0000)
or by fax (800-728-9048) or online.
- You will receive a "master" Order
Form for use in ordering tests via fax. You will also receive
a HIPAA release to be signed by each patient.
- ALL INQUIRIES REGARDING TESTS PERFORMED,
PROCEDURES OR BILLING MUST BE ADDRESSED WITH DLS ONLY. You
must call DLS, not the lab's Patient Service Center (PSC),
to change or add to an order.
- As soon as an order is received, DLS will
generate the necessary laboratory requisition form and email
to your patient (or you if the patient does not have email access).
It contains fasting instructions
and the PSC address for the blood draw. If a kit is necessary,
we mail the kit to your patient which is usually received within
2-4 days.
- The patient should have our toll free number
(800-908-0000) with them when going to the PSC,
and call if they encounter any problems.
- DLS receives the blood test results within
24-48 hours and securely emails the results
to the patient. An
additional copy can be sent to a third party or Healthcare provider,
with a signed HIPAA release form. When you setup your account,
you may choose if you want to receive the results only, or sent
to both of you.
- As required by law, our medical director
will review the results and contact patient if there are any
critical values that need immediate attention.
- Special tests (hormone panels, amino acids,
etc.) usually take additional time for analysis. You can contact
DLS for an estimation of the time required to process these tests.
- Pricing
You may simply refer your patients to DirectLabs and
they receive the website price. However, when you setup
an account with your credit card or checking account on file,
and you place the order, you receive an additional discount.

see our website (patient) prices. AFTER logging in with
your account code, username and password, it will show the Healthcare
Provider prices.

- Testing Procedures
Orders received by DLS before 2:00 pm cst, will be processed
that day. We use the patient's zip code to determine the nearest
draw site. An email is sent to your patient with the necessary
requisition form to take to the lab's Patient Service Center,
as well as the location choices. A HIPAA
release is included
in the package. Fasting
instructions are also included. Results
are emailed to the patient (usually within 24-48 hours after
blood draw) and a copy is sent to you also (with a signed HIPAA). You

Important!
Your must first register with Direct Labs (DLS) for
your patient and they receive
the necessary requisition prior to visiting a Patient
Service Center (PSC).
The PSC will not draw their blood without the DLS paperwork. |
