You
are referred to the Patient Service Center (PSC) draw site
of a major clinical laboratory near you. (Lab
Locations) We
provide the necessary order. You must have the orders/requisition
from DLS prior to going to the lab's PSC.
Confidential
results e-mailed directly to you within 24-48 hours. If you
would like your healthcare provider to receive a copy also, please
sign a HIPAA and
fax toll-free 1-800-728-9048.
DLS has contracts with major CLIA-certified clinical laboratories.
The labs have Patient Service Centers or draw centers, staffed by
certified phlebotomists - experts in blood draws. There are over
5000 PSC's nationwide, providing a location close to you.
Your zip code will help us determine which PSC is most convenient.
When your order is input, you will receive an auto email
confirmation to include an attached invoice. The
requisition is then produced and we email it to
you as an attachment to print and take
with you to the PSC. The second email has fasting and other important
instructions.
If you are unable to print
the requisition, please email us or call 1-800-908-0000 and
we will fax it to you or to the PSC location of your choice.
YOU MUST HAVE THE ORDERS (REQUISITION)
FROM DIRECTLABS BEFORE YOUR BLOOD CAN BE DRAWN.
Your blood is drawn at the PSC and then processed by that lab.
After your blood is drawn at the lab's PSC, the report will print
in our office within 24-48 hours. We will email
the results automatically along with a description of the tests. If
you would like a hard copy sent by mail, just let us know and
we will do so. If instructed by you, we will also fax a copy to your Physician
or healthcare provider, upon receipt of a HIPAA
release.
You may pay by credit card or check. The prices listed are all-inclusive
and no extra charges are added within the continental USA. Additional
shipping may be added for those outside the USA.
Please add $25.00 shipping & handling for all orders outside
the continental USA. After an order is processed, you may
cancel prior to blood draw with a refund equal to the price
of the test less $25 process & handling fee when cancelled
within six (6) months from date of order. There is no
cancellation refund after six months.
Because our prices are extremely discounted, we do not file insurance
claims. We will, however, prepare a HCFA for you to file,
at a charge of $15. The only diagnostic code that we use
is V70.0 for "wellness." Only your physician
can prepare and file claims with other reimbursable diagnostic
codes.
