STEP 1:Request an Initial Health Evaluation with Test Recommendations using our secure online appointment system (see pricing info below). Click here for Appointment System. Once we receive your request, we will send you both a Health History Form and a Metabolic Assessment Form.
STEP 2: E-mail us back the completed forms.
After evaluating your forms, we will send an e-mail with recommended test panels and nutritional tips specific to your case.
STEP 3: To order blood work, first time users must create a MyDLS account by clicking on the "Register" link above. Your secure MyDLS account is where you will print your requisition, submit HIPAA release forms, and access your results. If you need any assistance, please call 1-800-908-0000 and give the code EQL.
Pricing Information:
EQ Evaluation & Consultation Fees:$125 for up to 30 minutes.
Initial Health Evaluation & Email response of Test Recommendations: 30 minutes = $125
Lab Evaluation & Phone Consultation: 60 minutes: 30 min. evaluation of your lab tests results and 30 min. of phone consultation time = $250
