they need to take control of their own health, and
direct-to-consumer testing programs allow greater

refer to DLS for this reason, and because we refer
to the same labs that physicians and hospitals use,
which are CLIA-certified and regulated by the appropriate
governmental agencies.
An appointment is usually
not required at most Patient Service Centers. The
requisition form we send to you will indicate if
it is necessary. But remember, fasting is recommended
for many tests.
It depends on the test you
are taking. It
is recommended that you avoid food and liquids
for a minimum of 8 hours when testing for Glucose,
Cholesterol, and Triglycerides. Water or black
coffee are permitted during your fast.
You can expect to receive
the results by e-mail within 28-48 hours
after your blood is drawn for serum tests. Results
from the specialty labs on hair, saliva, urine,
and stool may take a week or longer. If you

Absolutely. We respect your
privacy and maintain confidentiality. You are the
only one who receives the results unless you specifically
direct us otherwise, with a signed
HIPAA release.
Not even your insurance company will obtain results
from DLS and/or associates.
Yes. All test results include
the normal reference ranges, and abnormalities
indicated. We recommend you seek a healthcare
provider to discuss results out side normal ranges.

We deal directly, confidentially
with you and send the results to you only. We
do recommend that you take a copy of the results
with you when you visit your Doctor. Or
you may complete and return (submit, fax or mail)
this HIPAA release if you do want
a third party to receive the results as well.
Abnormalities should be considered
an early warning, but do not necessarily mean you
have an illness or disease. We strongly recommend
you discuss the results with your Doctor for evaluation,
further testing, and diagnosis.
Who
can I contact if I have questions about the results?
Questions regarding abnormal
results should be discussed with your Doctor. You
may call or write DLS with any general questions
regarding your results.
DLS does not file insurance
claims. Some insurance plans have a "wellness"
or "prevention" benefit and you should
contact your carrier for that information. We can
provide you with the CPT codes so you may file for
reimbursement.
Medicare only reimburses the
laboratory that performs the analysis. Medicare
patients must have a Doctor's order by a Physician
with a Medicare number, and the diagnostic code
accepted by Medicare.
Certified laboratories have
Patient Service Centers throughout the United
States where patients have their blood drawn.
These are staffed by licensed Phlebotoists and
the same facilities referred to by Doctors.
DLS uses only
nationwide CLIA-certified Medical Reference Laboratories
with Patient Service Centers close to where you
live or work for the blood draw. Special arrangements
can be made for an area without a facility.
You may
order by phone, fax or online. We then refer you to the laboratory
with the Patient Service Center most convenient to home or
office, where the blood is drawn by a certified phlebotimist.
The results are received by our office generally within
24-48 hours and e-mailed confidentially to you. For more information

I cancel my order?
After
an order has been processed, you may cancel prior to the blood
draw with a refund equal to the price of the test less $25
cancellation fee if cancelled within six (6) months from
date of order. There
