What is the Wellness Program?

DLS designed a Wellness Program for Louisiana State and School system employees who are Plan members (EPO and PPO) covered by the Louisiana Office of Group Benefits. The components include:

a.      The Comprehensive Wellness Profile™, a thorough blood test with over 50 results

b.       Prostate (PSA) for men 45+

c.      A complete urinalysis, and

d.     The Health Risk Assessment report

Because of the volume-based discount prices DLS receives from the major laboratories, you can take advantage (at NO cost) of this package valued at over $500.

Who is Eligible

The Well-Adult Care benefit is available to employees, covered dependents, and retirees enrolled in the PPO or EPO plan.  Expenses are not subject to the annual deductible, but limited to a maximum benefit of $200.00 by age category:

a.      Age 16 until Age 40 - 1 physical every three years

b.      Age 40 until Age 50 - 1 physical every two years

c.       Age 50 and over - 1 physical every year

What is the Cost?

The DLS Wellness Program is NO COST to eligible plan members described above.  You have paid for this benefit through your premiums.  Do not miss out on this important element of your health insurance coverage. 

May I Use My Own Doctor?

You may now skip the Doctor's visit and go straight to the lab! We will send you the necessary requisition to go to the closest CLIA-licensed clinical reference laboratory, which saves time and money. Your Wellcare benefit may be used through any licensed physician. There could be a substantial charge to you, however, for a similar test through a Physician not participating with DLS in this program.  Many plan members prefer to use their Wellcare benefit through the DLS Wellness Program because of the substantial value.  Although the confidential results of the test are mailed directly to you, we will send a copy to your personal Physician upon your request.

How To Participate

Call toll-free 1-800-908-0000 and press 221 to speak with Stephanie Ballard, the OGB representative who will help you determine eligibility.  After pre-qualification, you will be supplied with the address of the lab nearest you for your blood draw, and the necessary requisition form to take with you to the lab.
