Description: Estradiol, also known as E2, is the most active of the estrogens. For women, it is important to look at the relationship between estradiol and progesterone in evaluating menopausal symptoms such as hot flashes, mood disorders, and aging skin. In men, high levels of estradiol are associated with abdominal fat, enlargement of the prostate and cardiovascular risk. In both men and women, low levels of estradiol are associated with osteoporosis.
Measuring the circulating levels of estradiol is important for assessing the ovarian function and monitoring follicular development for assisted reproduction protocols. Estradiol plays an essential role throughout the human menstrual cycle. Elevated estradiol levels in females may also result from primary or secondary ovarian hyperfunction. Very high estradiol levels are found during the induction of ovulation for assisted reproduction therapy or in pregnancy. Decreased estradiol levels in females may result from either lack of ovarian synthesis (primary ovarian hypofunction and menopause) or a lesion in the hypothalamus-pituitary axis (secondary ovarian hypofunction). Elevated estradiol levels in males may be due to increased aromatization of androgens, resulting in gynecomastia.

Description: Total testosterone: A hormone that causes male characteristics. The blood level is used by men to investigate abnormal sexual development and sexual dysfunction. Small amounts are produced in women's ovaries and levels are tested to evaluate virilization. A drop in testosterone levels can contribute to moodiness, weight gain, loss of muscle mass, and diminished libido.
Testosterone, Free (Dialysis): Measures the amount of testosterone not bound to any other chemicals in the blood and may be used to assist in the evaluation of testicular function when obesity, cirrhosis or thyroid disorders may alter testosterone binding proteins.
Both men and women can have health problems because of low or high levels of free testosterone. Women with high levels of free testosterone may have polycystic ovary syndrome (PCOS), a condition marked by infertility, lack of menstruation, acne, and excessive hair growth, especially on the face. Men with low levels of free testosterone can lose their sex drive, suffer bone loss, or become infertile.

PSA: The prostate specific antigen (PSA), is a protein made only in the prostate gland. PSA is produced by normal, abnormal and cancerous prostatic tissue. The PSA blood test is an accurate measure of this amount. The theory is that cancer causes more of the protein to be made and leaked into the blood than normal prostate tissue, so PSA is now used for assisting in the diagnosis and monitoring of prostatic carcinoma.
Insulin: Insulin is the hormone that enables cells to take in glucose. Without insulin, glucose can't get into the cells and it stays in the bloodstream. With too little insulin, blood sugar remains higher than normal (a condition known as hyperglycemia) and cells can't get the energy they need. With too much insulin, blood sugar decreases (hypoglycemia), causing symptoms such as sweating, trembling, lightheadedness, and in extreme cases, shock.

Patient Instructions: Do not take oral DHEA 48 hours before, or any estrogen or pregnenolone 72 hours before, your first collection. Do not skip doses of birth control for this test unless instructed by your practitioner. Any other hormones taken at night (including oral progesterone) should be taken after the bedtime sample. Generally, hormone creams or gels can be taken as usual during the test. All hormones taken in the morning should not be taken until after sample #4. For patches, pellets and injections, collect midway between doses.
